About Bright Leaf Senior Resources
At Bright Leaf Seniors, we believe every older adult deserves dignity, safety, and support. Our mission is to serve low‑income and disabled senior citizens across Colorado with the essentials they need to thrive at home.
We provide:
-
Free home services to keep seniors safe and comfortable
-
Food assistance for those facing hunger or limited mobility
-
Statewide support reaching seniors in every corner of Colorado
Eligibility: Seniors age 55 and older with an annual income of $45,000 or less.
We are here for our senior community and the most vulnerable among us—those who have given so much and now deserve care, compassion, and stability. Whether it’s repairing a home, delivering groceries, or simply offering hope, Bright Leaf Seniors stands beside Colorado’s aging population to ensure no one is left behind.

New Requirement: To be eligible for our free home repair program, seniors must first enroll in Colorado PACE.
-
Why? Because your healthcare is just as important as your home care. For seniors to succeed, their homes need safety measures and their health needs must be addressed.
-
How to Apply:
-
Call Colorado PACE and set up your participation.
-
After enrolling, complete the application form at the bottom of this page.
-
Your health and your home are both very important to us. By working together with Colorado PACE, we can ensure seniors receive the care and resources they deserve.
Most seniors prefer to age in place, and the Colorado PACE program is designed to help them do exactly that. Colorado PACE provides nursing home–eligible seniors with comprehensive healthcare services that allow them to live independently in their own homes and communities for as long as it is safely possible.
Colorado PACE joins a respected group of palliative care and hospice partners with deep roots in the Colorado healthcare community – including The Denver Hospice, Pathways Hospice in Fort Collins, Pikes Peak Hospice in Colorado Springs, and The Colorado Visiting Nurse Association. Together, they extend the continuum of care available to seniors across the Front Range and the Denver Metro Area.
Eligibility for Colorado PACE:
-
Be at least 55 years of age
-
Live in the Colorado PACE designated service area
-
Meet the state’s requirements for needing nursing facility level of care
-
Be able to live in a community setting without jeopardizing your health or safety

Bright Leaf Seniors is excited to introduce our newest partner: the Colorado Visiting Nurse Association (CVNA) CAPABLE Program. Click the Image below.
As the first community-based home health organization to adopt the CAPABLE approach, Colorado Visiting Nurse Association are leaders in fostering elder independence in Colorado. Our success stories speak volumes, with countless clients experiencing significant improvements in their daily lives. Our CAPABLE program includes ten home visits tailored to your needs: six with an occupational therapist and four with a registered nurse. Each session is designed to ensure your home environment is as safe and supportive as possible, laying the foundation for your continued independence.

The GrandCare Alliance Program
The GrandCare Alliance supports senior grandparents who are raising their grandchildren, providing them with resources and community care.
Right now, there are 20 children in the program — and the number is growing. Many of these kids have little or no parent involvement, and this season we want to make sure they feel loved and supported.
We’re asking for holiday gifts, donations and essentials for the children in this program. To make it easy, we’ve created an Amazon Wish List where you can sponsor a child by selecting items like clothing, shoes, and toys.
💙 Your generosity will mean the world to these families. The link to the wish list is at the end — thank you for helping us bring joy to children and their grandparents this holiday season. Amazon wish list
